Patients are seen by appointment only. When you make your appointment with our receptionist, please indicate the reason for the appointment so that we may allow an appropriate amount of time to provide you with the best care.

We will make every effort to see you at your scheduled appointment time; however, from time to time emergencies may occur and routine office visits may be delayed.

Same-Day Appointments

We offer same-day appointments every day, so if you are having an urgent problem you may call the office to schedule a same-day appointment.


If you need to cancel an appointment, please do so at least 48 hours in advance. We reserve the right to charge a fee for missed appointments.

We have compiled a list of items that will help our office provide you with the best care possible. Please let us know if you have questions or concerns, or if there is anything we can do to make your visit more productive, educational and comfortable.

Please bring with you:

  • Your past medical history
  • A list of (or the actual) over-the-counter medications you are currently taking
  • A list of (or the actual) prescription medications you are currently taking and the dosage instructions
  • Insurance information/card. Please confirm that we accept your insurance prior to your visit.
  • The Drug Formulary Lists provided by your insurance company.


FOR TRUE EMERGENCIES, CALL 911 OR GO DIRECTLY TO THE ER. Once you have received emergency treatment, please notify our office in a timely fashion.

When calling our office during regular hours, please advise the receptionist if the call is urgent. Outside of office hours, there is a physician on call for emergencies 24 hours a day, 365 days a year. Please be advised that this service is provided only for emergencies and only for established patients of our practice.

Prescription refills, appointments, referrals and other routine matters should be addressed during regular office hours when our staff is available. To reach the physician on call after normal business hours, call the office phone number.

After-hours care is available by calling one of the office phone numbers shown below. You will be transferred to the Medical Bureau and a provider will be contacted to respond to your medical needs.
